Summary

Both drugs suppress immune function, so using them together can deepen lymphocyte and neutrophil depletion and raise the risk of serious infection. Mitoxantrone is also used in multiple sclerosis, and its effects on bone marrow can persist for weeks after the last dose.

What this interaction means in plain words

Mitoxantrone and Siponimod can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Mitoxantrone belongs to Central Nervous System; Siponimod belongs to Central Nervous System.

Evidence level: Moderate

Why it happens

Additive immunosuppression: siponimod sequesters lymphocytes in lymphoid tissue via S1P receptor modulation, while mitoxantrone causes myelosuppression and lymphocyte depletion.

What to do

Avoid starting siponimod while mitoxantrone effects persist; check CBC with lymphocyte count before starting and monitor for infection. Consider the long duration of mitoxantrone's effect when switching.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
