Summary
With nonselective beta-blockade, norepinephrine's alpha-mediated vasoconstriction is unopposed, which can cause marked hypertension with reflex bradycardia. Its beta-1 mediated cardiac stimulation is also blunted, altering the expected hemodynamic response.
What this interaction means in plain words
Nadolol and Norepinephrine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Why it happens
Nadolol blocks beta receptors, leaving alpha-adrenergic vasoconstriction by norepinephrine unopposed and blunting its cardiac stimulation.
What to do
Monitor blood pressure and heart rate continuously during norepinephrine infusion in patients on nadolol. Titrate cautiously and be prepared to adjust therapy for severe hypertension or bradycardia.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
