Summary
Saquinavir can prolong PR and QT intervals, and beta-blockers slow AV conduction, so conduction disturbances and bradyarrhythmias may be more likely. Ritonavir-boosted saquinavir may also raise nebivolol levels via CYP2D6 inhibition.
What this interaction means in plain words
Nebivolol and Saquinavir can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Why it happens
Additive effects on cardiac conduction, with possible CYP2D6 inhibition from boosting ritonavir increasing nebivolol exposure.
What to do
Use an alternative antihypertensive or antiretroviral if possible; otherwise obtain a baseline ECG and monitor heart rate and PR interval.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
