Summary

Both agents can injure the kidney, and oral neomycin is absorbed to a small degree, which may be greater with damaged gut mucosa or renal impairment. Together with tenofovir's proximal tubular toxicity, this may add to the risk of renal impairment and aminoglycoside ototoxicity.

What this interaction means in plain words

Neomycin and Tenofovir df can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

Additive nephrotoxic effects on the renal tubule, with possible reduced renal clearance of tenofovir when renal function declines.

What to do

Monitor serum creatinine and electrolytes during combined use and keep neomycin courses short. Reduce or stop one agent if renal function worsens.
