Summary

Oral neomycin can be absorbed in small amounts, especially with impaired gut mucosa or renal impairment, and aminoglycosides have intrinsic neuromuscular blocking activity. This can prolong or intensify vecuronium paralysis and delay recovery of spontaneous breathing, with a risk of apnea.

What this interaction means in plain words

Neomycin po and Vecuronium can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Aminoglycosides reduce presynaptic acetylcholine release and postsynaptic sensitivity, adding to nondepolarizing neuromuscular blockade.

What to do

Monitor neuromuscular function with train-of-four and ensure ventilatory support is available until full recovery. Reduce vecuronium dosing as needed and be especially careful in renal impairment or bowel disease.

What to ask your pharmacist
