Summary

Colistin can itself cause neuromuscular blockade and may potentiate vecuronium. Prolonged paralysis and respiratory failure are possible.

What this interaction means in plain words

Colistin and Vecuronium can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Colistin belongs to Anti Bacterial; Vecuronium belongs to Musculoskeletal.

Evidence level: Moderate

Why it happens

Colistin reduces presynaptic acetylcholine release and stabilizes the postsynaptic membrane, adding to nicotinic blockade by vecuronium.

What to do

Avoid co-administration where possible; otherwise reduce vecuronium dose and monitor with train-of-four. Have ventilatory support available, as reversal may be incomplete.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
