Summary
Ponesimod lowers heart rate after initiation, and neostigmine, a cholinesterase inhibitor, increases vagal tone and can cause bradycardia. Together they may cause additive bradycardia or conduction block.
What this interaction means in plain words
Neostigmine and Ponesimod can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Neostigmine belongs to Central Nervous System; Ponesimod belongs to Central Nervous System.
Why it happens
Additive negative chronotropic effects: S1P1 receptor modulation of atrial pacing plus cholinesterase inhibition-mediated vagal stimulation.
What to do
Monitor heart rate and ECG, particularly during ponesimod initiation or when neostigmine is given. Seek cardiology advice before starting ponesimod in patients receiving heart-rate-lowering agents.
