Summary
Neostigmine inhibits acetylcholinesterase and is used to reverse non-depolarizing blockade by rocuronium, so the two drugs have directly opposing effects at the neuromuscular junction. Reversal is intended but may be incomplete if given when blockade is deep, and residual paralysis can occur.
What this interaction means in plain words
Neostigmine and Rocuronium can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Neostigmine belongs to Central Nervous System; Rocuronium belongs to Musculoskeletal.
Why it happens
Neostigmine increases acetylcholine at the neuromuscular junction, competitively overcoming rocuronium's nicotinic receptor antagonism.
What to do
Confirm some spontaneous recovery with quantitative train-of-four monitoring before giving neostigmine, and dose it accordingly. Give an antimuscarinic such as glycopyrrolate to limit bradycardia, or consider sugammadex.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
