Summary

Tolvaptan is a CYP3A4 substrate, but nifedipine is only a weak inhibitor, so any rise in tolvaptan exposure is expected to be modest. Increased tolvaptan effect could cause excess aquaresis, thirst or rapid sodium rise.

What this interaction means in plain words

Nifedipine and Tolvaptan have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Nifedipine belongs to Cardiovascular; Tolvaptan belongs to Endocrine.

Evidence level: Low

Why it happens

Weak CYP3A4 inhibition by nifedipine may slightly reduce tolvaptan clearance.

What to do

No dose change is generally needed. Monitor serum sodium and fluid status after starting tolvaptan.
