Summary
Nirmatrelvir inhibits CYP3A4 and may substantially increase quetiapine exposure, causing sedation, hypotension and QT prolongation. The effect is relevant because nirmatrelvir is given with ritonavir.
What this interaction means in plain words
Nirmatrelvir and Quetiapine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Nirmatrelvir belongs to Anti Virals; Quetiapine belongs to Central Nervous System.
Why it happens
CYP3A4 inhibition reduces the first-pass and systemic metabolism of quetiapine.
What to do
Reduce quetiapine to about one sixth of the usual dose during the antiviral course, or temporarily hold it where feasible. Monitor sedation, blood pressure and ECG, and restore the dose after the interaction resolves.
What to ask your pharmacist

- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
