Summary
Thioridazine blocks alpha-adrenergic receptors and can blunt the pressor response to norepinephrine, and it also prolongs the QT interval while catecholamines are proarrhythmic. The combination can make blood pressure control harder and increase the risk of ventricular arrhythmias.
What this interaction means in plain words
Norepinephrine and Thioridazine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Why it happens
Alpha-1 adrenergic antagonism by thioridazine opposing norepinephrine vasoconstriction, plus additive proarrhythmic effects.
What to do
Titrate norepinephrine to blood pressure response, as higher doses may be needed. Use continuous ECG monitoring and correct potassium and magnesium.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
