Summary

Both drugs can prolong the QT interval, so combined use may increase the risk of torsades de pointes. The risk is greater with bradycardia, hypokalemia, hypomagnesemia or existing heart disease.

What this interaction means in plain words

Nortriptyline and Pasireotide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Nortriptyline belongs to Central Nervous System; Pasireotide belongs to Endocrine.

Evidence level: Moderate

Why it happens

Additive QT prolongation; pasireotide also causes bradycardia, which adds to arrhythmic risk.

What to do

Obtain a baseline ECG and monitor it after starting the combination, and correct potassium and magnesium before and during treatment. Consider an alternative antidepressant in patients with other QT risk factors.

What to ask your pharmacist
