Summary

Primidone is metabolized to phenobarbital and is sedating, so combination with nortriptyline can increase drowsiness and psychomotor impairment. Phenobarbital derived from primidone induces hepatic enzymes and may lower nortriptyline exposure. Tricyclics may also lower the seizure threshold.

What this interaction means in plain words

Nortriptyline and Primidone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Nortriptyline belongs to Central Nervous System; Primidone belongs to Central Nervous System.

Evidence level: Moderate

Why it happens

Additive CNS depression plus enzyme induction by primidone and its metabolite phenobarbital.

What to do

Monitor sedation, seizure control and antidepressant response, and consider nortriptyline plasma level monitoring with dose adjustment.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
