Summary

Fluoroquinolones and systemic corticosteroids each raise the risk of tendinopathy, and together the risk of tendon inflammation and rupture is greater. Older patients and transplant recipients are at highest risk.

What this interaction means in plain words

Ofloxacin and Triamcinolone can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Ofloxacin belongs to Anti Bacterial; Triamcinolone belongs to Endocrine.

Evidence level: High

Why it happens

Additive pharmacodynamic tendon toxicity, with both drugs impairing tenocyte function and collagen integrity.

What to do

Where possible choose a non-fluoroquinolone antibiotic in patients receiving corticosteroids, especially older adults. Stop ofloxacin and rest the limb at the first sign of tendon pain or swelling.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
