Summary

NSAIDs such as tolfenamic acid can reduce the antihypertensive effect of angiotensin receptor blockers and impair renal function. In volume-depleted, elderly or renally impaired patients this can cause acute kidney injury and hyperkalemia.

What this interaction means in plain words

Olmesartan and Tolfenamic acid can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Olmesartan belongs to Cardiovascular; Tolfenamic acid belongs to Musculoskeletal.

Evidence level: Moderate

Why it happens

NSAID inhibition of renal prostaglandins reduces renal perfusion, which with ARB blockade of efferent arteriolar tone impairs glomerular filtration and potassium excretion.

What to do

Use the lowest effective NSAID dose for the shortest time and ensure hydration. Monitor blood pressure, serum creatinine and potassium.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
