Summary

Oxymetazoline is an alpha-adrenergic agonist, and in patients on a nonselective MAOI such as tranylcypromine, sympathomimetic effects can be exaggerated. This may cause a hypertensive reaction with headache and, rarely, hypertensive crisis. Systemic absorption from nasal use is limited, which makes serious reactions less likely but still possible.

What this interaction means in plain words

Oxymetazoline intranasal and Tranylcypromine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

MAO inhibition increases neuronal norepinephrine stores, so adrenergic agonists produce an enhanced pressor response.

What to do

Avoid intranasal decongestants where possible and use saline or other non-sympathomimetic measures instead. If used, give at the lowest dose for a short time and monitor blood pressure.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
