Summary
Pyridostigmine inhibits acetylcholinesterase and antagonizes non-depolarizing blockers such as pancuronium, so blockade may be shortened or resisted. Patients with myasthenia gravis on pyridostigmine are also unusually sensitive to non-depolarizing agents, making the net response unpredictable.
What this interaction means in plain words
Pancuronium and Pyridostigmine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Pancuronium belongs to Musculoskeletal; Pyridostigmine belongs to Central Nervous System.
Why it happens
Pharmacodynamic antagonism: increased acetylcholine at the neuromuscular junction competes with pancuronium.
What to do
Use train-of-four monitoring and titrate pancuronium cautiously, starting with a reduced dose in myasthenic patients. Alert the anesthesia team to pyridostigmine use before surgery.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
