Summary

Colistin (polymyxin E) has neuromuscular blocking effects and can markedly potentiate pancuronium. Prolonged paralysis and respiratory arrest are possible, and reversal with anticholinesterases or calcium may be incomplete.

What this interaction means in plain words

Colistin and Pancuronium can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Colistin belongs to Anti Bacterial; Pancuronium belongs to Musculoskeletal.

Evidence level: Moderate

Why it happens

Additive neuromuscular blockade, as colistin inhibits presynaptic acetylcholine release and has postsynaptic effects.

What to do

Avoid combined use where possible; if needed, reduce pancuronium dose and monitor with train-of-four. Ensure ventilatory support is available, especially in patients with renal impairment.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
