Summary
Magnesium given as part of a bowel preparation can potentiate nondepolarizing neuromuscular blockade. This may prolong paralysis and delay recovery after surgery. Effect depends on magnesium absorbed, which is higher with renal impairment.
What this interaction means in plain words
Pancuronium and Sodium sulfate / ?magnesium sulfate / potassium chloride can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Magnesium reduces presynaptic acetylcholine release and decreases postsynaptic sensitivity, adding to nondepolarizing blockade.
What to do
Monitor neuromuscular function with train-of-four and consider reduced pancuronium dosing in patients exposed to magnesium. Check serum magnesium in renal impairment.
