Summary
Corticosteroids given with nondepolarizing neuromuscular blockers, especially in prolonged ICU use, are associated with acute myopathy. This may present as prolonged weakness and difficulty weaning from ventilation. Risk is mainly with high-dose or prolonged systemic exposure.
What this interaction means in plain words
Pancuronium and Triamcinolone acetonide injectable can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Steroid-induced muscle catabolism and denervation-like changes combine with prolonged neuromuscular blockade, causing myopathy.
What to do
Use the lowest effective blocker dose and shortest duration, with train-of-four monitoring. Assess for weakness and consider creatine kinase if prolonged weakness occurs.
What to ask your pharmacist
