Summary
Nonselective beta-blockers such as penbutolol impair cellular potassium uptake and can slightly raise serum potassium. Combined with potassium citrate, this may modestly increase hyperkalemia risk, mainly with renal impairment.
What this interaction means in plain words
Penbutolol and Potassium citrate have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.
Why it happens
Beta-2 blockade reduces Na+/K+-ATPase-driven cellular potassium uptake, adding to the potassium load.
What to do
Monitor serum potassium in patients with renal impairment or diabetes or those taking other potassium-raising drugs.
