Summary
Penbutolol is a nonselective beta-blocker that opposes the bronchodilating effect of vilanterol and can provoke bronchospasm, especially in patients with asthma or COPD. This may reduce the efficacy of the inhaler and worsen airway obstruction.
What this interaction means in plain words
Penbutolol and Umeclidinium-vilanterol can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Why it happens
Pharmacodynamic antagonism at beta-2 adrenergic receptors by a nonselective beta-blocker.
What to do
Avoid nonselective beta-blockers in patients with reactive airway disease and use a cardioselective agent such as bisoprolol or metoprolol if a beta-blocker is needed. Monitor respiratory symptoms and lung function.
