Pimozide blocks dopamine D2 receptors and opposes the effect of the dopamine agonist pramipexole. This can worsen Parkinson symptoms or reduce control of restless legs syndrome, and the combination also adds to extrapyramidal risk.
What this interaction means in plain words
Pimozide and Pramipexole can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Pimozide belongs to Central Nervous System; Pramipexole belongs to Cardiovascular.
Why it happens
Pharmacodynamic antagonism at dopamine D2/D3 receptors.
