Summary
Telmisartan reduces aldosterone-mediated potassium excretion, and potassium citrate adds a potassium load. Together they can cause clinically significant hyperkalemia, particularly in renal impairment, diabetes or older age.
What this interaction means in plain words
Potassium citrate and Telmisartan-amlodipine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Potassium citrate belongs to Gastrointestinal Tract; Telmisartan-amlodipine belongs to Cardiovascular.
Why it happens
ARB-induced aldosterone suppression decreases renal potassium excretion while the supplement increases potassium intake.
What to do
Check serum potassium and renal function before and within the first weeks of combined use, then periodically. Avoid the combination in patients with reduced renal function or baseline high potassium.
