Summary

Systemic corticosteroids given with non-depolarizing neuromuscular blockers, especially during prolonged ICU use, are associated with acute quadriplegic myopathy. Corticosteroids may also alter the intensity or duration of neuromuscular blockade, so recovery can be delayed or the response variable.

What this interaction means in plain words

Prednisolone and Rocuronium can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Prednisolone belongs to Endocrine; Rocuronium belongs to Musculoskeletal.

Evidence level: Moderate

Why it happens

Additive effects on skeletal muscle (steroid-induced myopathy plus prolonged neuromuscular blockade and denervation-like changes); the precise mechanism is not fully established.

What to do

Use the shortest effective duration of neuromuscular blockade and monitor depth of block with train-of-four. Assess for persistent weakness and check creatine kinase if myopathy is suspected.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
