Summary
Prilocaine can cause cardiovascular depression when absorbed systemically, and propranolol may add to hypotension and bradycardia. Systemic exposure is greater with large-dose infiltration or regional blocks, and epinephrine-containing preparations may produce hypertension with reflex bradycardia in patients on nonselective beta-blockers.
What this interaction means in plain words
Prilocaine and Propranolol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Prilocaine belongs to Central Nervous System; Propranolol belongs to Cardiovascular.
Why it happens
Additive cardiac depressant effects, with unopposed alpha effects of any co-administered epinephrine during nonselective beta-blockade.
What to do
Monitor blood pressure and heart rate when larger anesthetic doses are used, and limit the dose of epinephrine-containing prilocaine. Routine small dental doses generally need no change.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?

How serious is the interaction between Prilocaine and Propranolol?
MediHelp rates it moderate, on a four-level scale (minor, moderate, major, contraindicated) based on published labels and references. What to do, in brief: Monitor blood pressure and heart rate when larger anesthetic doses are used, and limit the dose of epinephrine-containing prilocaine. Routine small dental doses generally need no change.
