Summary

Primidone, and its active metabolite phenobarbital, induce hepatic enzymes that convert tamoxifen to its metabolites. Exposure to tamoxifen and its active metabolite endoxifen may fall, which could weaken anti-estrogen effect.

What this interaction means in plain words

Primidone and Tamoxifen can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Primidone belongs to Central Nervous System; Tamoxifen belongs to Endocrine.

Evidence level: Moderate

Why it happens

Induction of CYP3A4 and CYP2C9 by primidone/phenobarbital increases tamoxifen clearance.

What to do

Prefer a non-inducing anticonvulsant where possible. If the combination is needed, monitor for loss of tamoxifen efficacy and review the endocrine therapy plan with the oncologist.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
