Summary
Probenecid decreases the renal clearance of ganciclovir, the active form of valganciclovir, and increases systemic exposure. This raises the risk of ganciclovir toxicity, notably myelosuppression and neurotoxicity including seizures.
What this interaction means in plain words
Probenecid and Valganciclovir can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Probenecid belongs to Genitourinary; Valganciclovir belongs to Anti Virals.
Why it happens
Probenecid inhibits renal tubular secretion of ganciclovir via organic anion transporters.
What to do
Avoid the combination where possible; if used, monitor complete blood counts and neurologic status and consider reducing the valganciclovir dose.
