Summary
Procainamide is a class IA antiarrhythmic with clear proarrhythmic and QT-prolonging effects, and sertraline can add a smaller further effect. The combination may increase the risk of torsades de pointes, especially with renal impairment, bradycardia or electrolyte abnormalities.
What this interaction means in plain words
Procainamide and Sertraline can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Why it happens
Additive prolongation of cardiac repolarization; sertraline may also modestly inhibit CYP2D6, which contributes to procainamide metabolism.
What to do
Prefer an alternative antidepressant with less QT effect if procainamide is required. If combined, monitor ECG and serum potassium and magnesium and watch procainamide and NAPA levels.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
