Summary
Procainamide prolongs the QT interval and vilanterol, as a beta-2 agonist, can add to cardiac repolarization effects and cause hypokalemia and tachycardia. Together they may increase the risk of ventricular arrhythmias including torsades de pointes.
What this interaction means in plain words
Procainamide and Umeclidinium-vilanterol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Additive QT prolongation and pro-arrhythmic effects from a class IA antiarrhythmic and a beta-2 agonist, with possible beta-agonist-induced hypokalemia.
What to do
Obtain a baseline ECG and monitor QTc and serum potassium and magnesium, correcting deficits. Consider an alternative if the patient has other QT risk factors.
What to ask your pharmacist

- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
