Summary

Procarbazine has weak monoamine oxidase inhibiting activity, and S-adenosylmethionine (SAMe) may raise central serotonin and other monoamine activity. Together they may contribute to serotonin syndrome, with agitation, hyperthermia, tremor, clonus and autonomic instability.

What this interaction means in plain words

Procarbazine and Same can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Evidence level: Low

Why it happens

Additive serotonergic effect from MAO inhibition by procarbazine and increased monoamine synthesis or availability with SAMe.

What to do

Avoid SAMe in patients receiving procarbazine and stop it before starting therapy. If it is taken, monitor for signs of serotonin toxicity and discontinue SAMe if they appear.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
