Summary
Both phenothiazine and ziprasidone antipsychotics can prolong the QT interval. Combining them raises the risk of torsades de pointes and sudden cardiac death, and ziprasidone labeling contraindicates its use with other QT-prolonging drugs.
What this interaction means in plain words
Promazine and Ziprasidone are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Why it happens
Additive blockade of cardiac hERG potassium channels causing delayed repolarization.
What to do
Do not use together; use a single antipsychotic. When switching, allow the first drug to wash out and check the ECG.
