Summary
Warfarin lowers protein C and protein S early in therapy, before it reduces clotting factors, producing a transient hypercoagulable state. This can cause warfarin-induced skin necrosis, especially in patients with congenital protein C deficiency, so protein C replacement is used as cover during initiation.
What this interaction means in plain words
Protein c and Warfarin can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Protein c belongs to Cardiovascular; Warfarin belongs to Cardiovascular.
Why it happens
Warfarin rapidly depletes the short-half-life vitamin K-dependent protein C ahead of procoagulant factors, causing a transient prothrombotic imbalance.
What to do
Continue protein C replacement while starting warfarin until stable therapeutic anticoagulation is reached, and start warfarin at a low dose. Monitor INR and the skin closely.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
