Summary

Protriptyline is a tricyclic antidepressant with anticholinergic and arrhythmogenic properties. Tricyclics given with neuromuscular blockers during anesthesia have been associated with arrhythmias and blood pressure changes, but there is no robust evidence of a clinically important effect on blockade.

What this interaction means in plain words

Protriptyline and Vecuronium have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Evidence level: Low

Why it happens

The mechanism is not well established; additive autonomic and cardiac effects of the tricyclic are possible, with little direct effect on nicotinic blockade.

What to do

Monitor ECG and hemodynamics during anesthesia and use train-of-four monitoring to guide vecuronium dosing.

What to ask your pharmacist
