Summary

Both are antipsychotics with D2 antagonism, so combining them raises the risk of extrapyramidal symptoms and neuroleptic malignant syndrome. Zotepine also lowers the seizure threshold and prolongs the QT interval, and sedation and hypotension add together.

What this interaction means in plain words

Quetiapine and Zotepine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Evidence level: Moderate

Why it happens

Additive D2, histamine H1 and alpha-1 receptor blockade, with additive QT and seizure-threshold effects.

What to do

Avoid concurrent use of the two antipsychotics where possible. If cross-titrating, monitor for movement disorders, sedation, blood pressure and QTc.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
