Summary
NSAIDs can reduce the antihypertensive effect of ACE inhibitors and increase the risk of acute kidney injury and hyperkalemia. Risk is higher in older patients, those with volume depletion or existing renal impairment, and with concurrent diuretics.
What this interaction means in plain words
Quinapril and Sulindac can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
NSAID inhibition of renal prostaglandin synthesis reduces renal perfusion and sodium excretion, counteracting ACE inhibitor effects.
What to do
Monitor blood pressure, serum creatinine and potassium when starting or changing the NSAID, and keep patients hydrated. Use the lowest effective NSAID dose for the shortest time, or choose an alternative analgesic in high-risk patients.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
