Summary
Bowel preparations can cause fluid loss and electrolyte shifts such as hypokalemia and hypomagnesemia. These disturbances add to the QT-prolonging and proarrhythmic effect of quinidine and may precipitate torsades de pointes.
What this interaction means in plain words
Quinidine and Sodium sulfate / ?magnesium sulfate / potassium chloride can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Why it happens
Electrolyte disturbance from bowel cleansing adds to quinidine-induced prolongation of cardiac repolarization.
What to do
Check potassium, magnesium and calcium before the preparation and correct any abnormalities. Consider an ECG before and after the procedure in patients with additional arrhythmia risk, and ensure adequate hydration.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
