Summary
Corticosteroids given with non-depolarizing neuromuscular blockers, mainly during prolonged use in critically ill patients, have been associated with acute quadriplegic myopathy. Corticosteroids may also alter the duration of neuromuscular blockade.
What this interaction means in plain words
Rocuronium and Triamcinolone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Rocuronium belongs to Musculoskeletal; Triamcinolone belongs to Endocrine.
Why it happens
Not fully established; thought to involve combined effects on skeletal muscle, with corticosteroid-induced myosin loss and denervation-like changes after prolonged blockade.
What to do
Limit the duration and dose of neuromuscular blockade and corticosteroid exposure in intensive care patients. Monitor neuromuscular function and watch for prolonged weakness or raised creatine kinase.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
