Summary

Saquinavir, a boosted-protease-inhibitor component, strongly inhibits CYP3A4 and raises saxagliptin exposure, which can increase the risk of adverse effects of saxagliptin. Protease inhibitors may also worsen insulin resistance and glycemic control, partly opposing the effect of metformin.

What this interaction means in plain words

Saquinavir and Saxagliptin-metformin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Strong CYP3A4 inhibition reduces saxagliptin metabolism, while protease inhibitor-associated insulin resistance pharmacodynamically opposes metformin.

What to do

Limit saxagliptin to 2.5 mg once daily during coadministration with saquinavir. Monitor blood glucose and adjust antidiabetic therapy as needed.
