Summary

Sulfamethoxazole can enhance the effect of sulfonylureas and has been associated with hypoglycemia, sometimes severe. Risk is higher in elderly patients and in renal impairment.

What this interaction means in plain words

Sulfamethoxazole and Tolazamide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Sulfamethoxazole may displace the sulfonylurea from albumin and inhibit its CYP2C9-mediated metabolism, and it can itself stimulate insulin release.

What to do

Monitor blood glucose during co-trimoxazole therapy and lower the tolazamide dose if hypoglycemia occurs. Consider an alternative antibiotic in high-risk patients.

What to ask your pharmacist
