Summary
NSAIDs can reduce the blood pressure lowering effect of angiotensin receptor blockers. In elderly, volume-depleted or renally impaired patients the combination can cause acute kidney injury and hyperkalemia.
What this interaction means in plain words
Sulindac and Telmisartan can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Sulindac belongs to Musculoskeletal; Telmisartan belongs to Cardiovascular.
Why it happens
NSAIDs block prostaglandin-mediated afferent arteriolar dilation while the ARB dilates the efferent arteriole, reducing glomerular filtration pressure; both also impair potassium excretion.
What to do
Ensure adequate hydration and monitor serum creatinine and potassium within the first weeks of combined use and after dose changes. Avoid the combination in patients with volume depletion or advanced renal disease where possible.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
