Summary

Xipamide, a thiazide-like diuretic, can raise blood glucose and reduce the effectiveness of tolazamide. Its potassium-lowering effect may also impair insulin secretion.

What this interaction means in plain words

Tolazamide and Xipamide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Thiazide-like diuretics cause hypokalemia and reduced insulin release and sensitivity, antagonizing sulfonylurea action; increased renal clearance of tolazamide is not an established mechanism.

What to do

Monitor blood glucose and serum potassium after starting xipamide. Increase antidiabetic therapy as needed and correct hypokalemia.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
