Summary
Tolmetin may blunt the blood pressure lowering effect of trandolapril. Together they can also reduce renal function, particularly in older, volume-depleted or renally impaired patients, and may raise potassium.
What this interaction means in plain words
Tolmetin and Trandolapril can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Tolmetin belongs to Musculoskeletal; Trandolapril belongs to Cardiovascular.
Why it happens
NSAID inhibition of vasodilatory renal prostaglandins opposes ACE inhibitor effects and reduces glomerular perfusion.
What to do
Prefer a non-NSAID analgesic. If co-use is necessary, ensure adequate hydration and monitor blood pressure, serum creatinine and potassium after starting and periodically.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
