Summary

Tolvaptan is a vasopressin antagonist that is not itself a potassium-sparing agent, but labeling notes a possible rise in serum potassium with it. Combined with an ARB, which reduces aldosterone effect, potassium may rise further. Rapid fluid loss can also lower blood pressure and affect renal function.

What this interaction means in plain words

Tolvaptan and Valsartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Tolvaptan belongs to Endocrine; Valsartan belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

ARB-induced reduction in aldosterone-mediated potassium excretion adds to a tolvaptan-associated potassium rise; the pharmacodynamic effects are additive.

What to do

Monitor serum potassium and renal function after initiating or changing doses. Also watch volume status and blood pressure.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
