Summary

Ulipristal may inhibit intestinal P-glycoprotein, which could raise venetoclax exposure. Greater venetoclax exposure increases the risk of neutropenia and tumor lysis syndrome, but the effect is predicted from in vitro data.

What this interaction means in plain words

Ulipristal and Venetoclax can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

Possible P-glycoprotein inhibition by ulipristal reduces venetoclax efflux and raises its absorption.

What to do

Monitor blood counts and signs of venetoclax toxicity, and consider a venetoclax dose reduction if a P-gp inhibitor is used per labeling. Prefer a non-interacting alternative if feasible.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
