Summary
Voriconazole increases venetoclax exposure, raising the risk of tumor lysis syndrome and neutropenia, especially during dose ramp-up. After ramp-up the combination may be used only with a substantial venetoclax dose reduction.
What this interaction means in plain words
Venetoclax and Voriconazole are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Venetoclax belongs to Miscellaneous; Voriconazole belongs to Anti Fungals.
Why it happens
Strong CYP3A4 inhibition (and possible P-gp inhibition) reduces venetoclax clearance.
What to do
Avoid during initiation and ramp-up in CLL/SLL. After ramp-up, reduce venetoclax dose per labeling and monitor blood counts and for toxicity.
