Prasugrel's active metabolite binds irreversibly to the P2Y12 class of ADP receptors on platelets, inhibiting platelet activation and aggregation.

- Effient can cause significant, sometimes fatal, bleeding. Do not use in patients with active pathological bleeding or a history of TIA or stroke. In patients ≥75 years it is generally not recommended, except in high-risk patients (diabetes or prior MI). Do not start in patients likely to undergo urgent CABG; when possible discontinue at least 7 days before any surgery. Other bleeding risk factors: body weight <60 kg, propensity to bleed, and concomitant medications that increase bleeding risk. - History of prior transient ischemic attack or stroke
- Hypersensitivity (e.g., anaphylaxis) to prasugrel or any component of the product

- CABG-related bleeding risk is increased; discontinue at least 7 days before CABG if possible
