Indications
Mécanisme d'action
Ivacaftor is a CFTR potentiator that increases chloride transport by increasing the channel open probability (gating) of CFTR protein at the cell surface.
Posologie adulte habituelle (résumé de la notice)
Cystic fibrosis (adults and patients aged 6 years and older): 150 mg orally every 12 hours (300 mg total daily dose) with fat-containing food
Grossesse et allaitement
- Grossesse
- Unknown Limited human data (under 300 pregnancies); no harm in rats or rabbits at 5 to 11 times human exposure. EU label: as a precaution, preferable to avoid. Stopping may worsen cystic fibrosis, and CF teams often continue it, so decide with the CF specialist.
- Allaitement
- Safe Low milk levels and very low infant blood levels; no infant harm reported, and expert groups consider it probably safe. EU label: weigh benefits. LactMed: check the infant's liver tests and eyes (cataracts) periodically, and test for CF, as it can mask it.
Effets indésirables signalés
- Headache
- Upper respiratory tract infection
- Nasal congestion
- Nausea
- Rash
- Oropharyngeal pain
Précautions particulières
- Elevated transaminases: assess ALT/AST before starting, every 3 months in first year, then annually; interrupt dosing if ALT or AST greater than 5 times ULN.
- Hypersensitivity reactions including anaphylaxis reported postmarketing; discontinue and treat if serious signs develop.
- Intracranial hypertension reported with similar active ingredients; interrupt and refer if unusual headache or visual disturbances occur.
Informez votre pharmacien ou votre médecin en cas de grossesse ou d'allaitement, de maladie rénale ou hépatique, d'allergies, ainsi que de tout autre médicament ou complément que vous prenez.
