Indications
Mécanisme d'action
Leucovorin is a mixture of diastereoisomers of 5-formyl tetrahydrofolic acid. It does not require reduction by dihydrofolate reductase to take part in folate one-carbon reactions, so it counteracts the therapeutic and toxic effects of folic acid antagonists such as methotrexate.
Posologie adulte habituelle (résumé de la notice)
Rescue after high-dose methotrexate (normal elimination): 15 mg IV or IM every 6 hours for 60 hours (10 doses total), starting 24 hours after the start of methotrexate infusion
Grossesse et allaitement
- Grossesse
- Safe Leucovorin (folinic acid) is a form of folate; no animal studies, and the US label says use only if clearly needed. Folate itself is not harmful in pregnancy. It is usually given with methotrexate or fluorouracil, which can harm the fetus: follow the partner drug's advice.
- Allaitement
- Safe Folinic acid and its forms are normal components of breast milk. Alone it is not expected to harm the infant (US label: use caution). When given with methotrexate or fluorouracil, the partner drug decides: breastfeeding is usually stopped during chemotherapy.
Effets indésirables signalés
- Stomatitis
- Vomiting
- Diarrhea
- Nausea
- Leukopenia
- Alopecia
Contre-indications
- Severe hypersensitivity reaction to leucovorin (folinic acid), levoleucovorin, or folic acid, including anaphylaxis
Précautions particulières
- Hypersensitivity reactions including anaphylaxis and urticaria can occur; withhold or permanently discontinue based on severity
- Hypercalcemia due to calcium content: do not exceed 160 mg per minute IV
- Do not give intrathecally; may be harmful or fatal
Informez votre pharmacien ou votre médecin en cas de grossesse ou d'allaitement, de maladie rénale ou hépatique, d'allergies, ainsi que de tout autre médicament ou complément que vous prenez.
