Mécanisme d'action
Tiratricol is a naturally circulating metabolite of T3 that binds with high affinity to thyroid hormone receptors TRα and TRβ and has T3-like effects. It can enter MCT8-dependent cells without a functioning MCT8 transporter, so it can replace T3 in MCT8-dependent tissues.
Posologie adulte habituelle (résumé de la notice)
Peripheral thyrotoxicosis in MCT8 deficiency, body weight 10 kg or above (adults, adolescents, children, infants): start 350 micrograms daily orally or via feeding tube in 1 to 3 doses; increase by 350 micrograms every two weeks to maintenance; target serum T3 below midpoint of normal range for age.
Grossesse et allaitement
- Grossesse
- Unsafe Contraindicated in pregnancy (EU): it crosses the placenta and animal studies showed reproductive toxicity. Women who could become pregnant must use effective contraception. Used for MCT8 deficiency, which almost only affects males.
- Allaitement
- Unknown Not known if it passes into milk; no LactMed record. EU label: risk cannot be excluded; decide whether to stop breastfeeding or the drug. e-lactancia: limited compatibility (a thyroid hormone analogue); prefer a safer option or stop breastfeeding.
Effets indésirables signalés
- Hyperhidrosis
- Diarrhoea
- Irritability
- Anxiety
- Nightmares
Contre-indications
- Hypersensitivity to the active substance or any excipient.
- Hyperthyroidism for reasons other than MCT8 deficiency (e.g. Graves' disease).
- Pregnancy.
Précautions particulières
- Hypermetabolic signs and symptoms (hyperhidrosis, irritability, anxiety, insomnia, tachycardia, diarrhoea etc.) may occur at initiation or during titration; reduce dose if not resolved within 2 weeks.
- Use caution during titration in patients with cardiac disease.
- Tiratricol cross-reacts with T3 in immunoassays, giving unreliable results; LC/MS/MS is recommended.
Informez votre pharmacien ou votre médecin en cas de grossesse ou d'allaitement, de maladie rénale ou hépatique, d'allergies, ainsi que de tout autre médicament ou complément que vous prenez.
